Section 1: Basic Agreement

How did you hear about CBR?

I am aware that before I am approved to foster or adopt, I will have a home visit conducted by a CBR volunteer to determine that my home is a safe, positive environment for a rescued dog

I agree to a follow-up visit by a CBR volunteer within 30-60 days of adoption to confirm the dog’s well being and provide support if needed

CBR does not do same-gender adoptions unless both the dog in the home and the newly adopted dog are both over 5 years of age to minimize the risk of same sex aggression. Read more about our policy here. You must agree to this policy to proceed with this application

Animals in the home or yard (name, breed, gender, age, spayed or neutered)

Carolina Boxer Rescue requires that all dogs in the home are altered unless there are extenuating circumstances (for example, they are less than a year old, they have a medical condition that would preclude them from being put under anesthesia). If any of your dogs are not spayed or neutered, please explain.

Carolina Boxer Rescue requires that all cats and dogs of applicants are up to date on rabies and distemper vaccines and that all dogs are on year round heartworm medication. We will contact your vet to confirm this.

Name of Vet:

Vet Phone Number:

What brand of heartworm medication are your pets on?

All dogs adopted from Carolina Boxer Rescue must be kept up to date on rabies and distemper vaccines as well as remain on year-round heartworm preventative

I understand this and I agree to take my dog to the vet at least annually for these things

Have you ever had to rehome or give away a pet? If yes, please explain

Section 5: Boxer Knowledge, Training, and Preparedness

What makes you a good fit for a boxer?

Why do you feel a rescue dog would be the right choice for you?

Have you ever fostered or rescued a dog before? If yes, please explain how you acclimated and settled them into your home.

What behavior would you deem unacceptable?

Most rescue dogs may need help with housetraining, crate training, or behaviors like counter surfing, jumping, leash pulling, or destructive habits. What is your experience with these types of behaviors, and what training strategies would you use?

Are you willing to allow a 30-60 day adjustment period for your new dog to adapt to your home, your family and any other pets?

Do you understand that a dog will take time to adjust to its new surroundings and that certain behavioral issues might occur, especially if they are not properly decompressed and slowly introduced to other pets and household members?

Are you willing to work to correct these issues?

In the event of behavioral challenges, CBR requires adopters to work with a trainer prior to returning the dog. Do you agree to this?

Boxers can have emergency health issues that can range from a few hundred dollars to $5,000-$10,000. Do you have a plan in place for such emergencies? Please describe
